These apply to information (including personal data) that is collected through the HDNA website, which is hosted by Monash University. Monash University is committed to protecting the integrity of your personal data and safeguarding your privacy. These Website Terms and Conditions are to be read in conjunction with the Participant Information Consent Forms which describe how we process your personal data and how you may exercise rights in relation to the processing of your data.
2. WHAT TYPES OF INFORMATION MIGHT WE COLLECT THROUGH YOUR USE OF MONASH WEBSITES?
Information processed by Monash University may also include personal information about you collected from your use of this website, using cookies, as well as the details you provide directly to us. 2.1 Information automatically collected
A cookie is a small message given to your web browser by our web server. The browser stores the message in a text file and the message is then sent back to the server each time the browser requests a page from the server. Although it is possible to disable the acceptance of cookies by your web browser, it will not be possible to use the Map-HD Registry portal if you do so.
Monash websites use cookies to store information regarding your visit to the site to improve your experience with Monash. Information that may be collected via cookies includes your domain name, IP address, the date and time of your visit, the pages accessed, documents downloaded, and the type of browser used.
We use the following types of cookies (which you can customise in accordance with Cookies Consent):
- Essential cookies: these cookies are required for our website to operate correctly, and are therefore always turned on. They include, for example, cookies that enable you to navigate the website and access to secure areas of the website. If you do not accept essential cookies, our website may not function properly.
- Performance and analytical cookies: these cookies allow us to analyse non-personal data about how you and other visitors use our website to measure how it’s performing so we can constantly improve it. These cookies are shared with third party providers, but no personal data is included.
2.2 Information you voluntarily provide
Information that you voluntarily provide through the Monash websites may be collected through various means, including through the use of online forms (e.g., enrolment and registration forms), online surveys, payment facilities, online collaborative spaces and through alternative contact points (such as social media platforms). If you are not a Monash student or a Monash employee and you send us a message, the University will record your e-mail address. If personal data is collected through the Monash websites, every effort will be made to ensure that this is done by a sufficiently secure means.
Third parties wishing to access registry data agree to and follow the requirements detailed within the Map-HD Registry Data Sharing Policy and Procedure document. This document details the conditions and limitations of accessing the Map-HD Registry data, and the obligations which must be met and the terms by which they must agree to regarding data handling. Prior to receiving any data, all recipients of registry data are required to provide evidence that a Human Research Ethics Committee has approved the study for which they are seeking Map-HD Registry data.
